The document USCIS asks for is not the hospital one
This is the most consequential thing to know about Chinese birth records, and it is where most of the wasted effort goes. Births in mainland China since 1996 produce a medical certificate of birth (出生医学证明), issued by the hospital. For US immigration purposes, what is generally required instead is a notarial birth certificate (公证书) issued by a local notary public office (公证处), which attests to the facts of birth by reference to the medical certificate, the household register or the archives.
For a birth before 1996, or where the medical certificate no longer exists, the notary issues a factual certificate built from the hukou household register (户口簿) or police records instead. Either way, the notarial booklet is the document that travels.
Check whether you need a translation at all
Notarial booklets are very often issued bilingually, with an English translation bound in alongside the Chinese. If yours is one of those, you may not need a translation service — and we would rather tell you that here than take the order. Look at the booklet before you buy anything.

What USCIS requires
The requirement is the same for every country: under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note on your birth certificate is part of the document, so every one is translated.

Frequently asked questions
Do I need the hospital certificate or the notarial one?
For US immigration filings it is generally the notarial certificate from a notary public office. The hospital medical certificate of birth is what the notary works from, not what is usually filed.
