Registered at the ward, issued locally
A Nepali birth is registered with the local ward office of the municipality or rural municipality, and the certificate (janma darta praman patra) is issued there. Because registration is local, the paper and the layout vary between municipalities and decades.

Bikram Sambat, which does not convert by subtraction
Nepal dates official documents in Bikram Sambat, roughly 56 years and 8 months ahead of the Gregorian calendar. Unlike the Thai Buddhist Era, subtracting a fixed number does not work: the Nepali new year falls in mid-April, and the months run between 29 and 32 days according to astronomical calculation rather than a fixed pattern.
That means every BS date has to be looked up rather than computed, and a careless conversion lands on the wrong day and sometimes the wrong month. We convert each date individually and show both, so an office can check the original against the English.
What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.
