Seven categories, four grades
China issues a 残疾人证 — a disability certificate — recording one of seven categories: visual, hearing, speech, physical, intellectual, mental and multiple, each graded 1 to 4 with 1 the most severe.
So the grade runs the opposite direction from a percentage and a reader assuming a higher number means greater severity will invert the finding. We state the category and the direction of the scale every time.
Who issues it, and what it opens
The certificate is issued by the Disabled Persons' Federation at county level on the basis of a designated hospital's assessment, and it is the document that opens entitlements — allowances, tax relief, transport concessions, employment quota placement.
It is an entitlement card rather than a clinical record. Where a US office needs the diagnosis, the hospital assessment is the document to send alongside it.
The certificate is tied to the hukou
Entitlements attached to the certificate are administered by the locality where the holder's hukou is registered, so a person living elsewhere may hold a valid certificate and access little.

The chop, and the ID number
The certificate carries the issuing federation's chop, a red seal, which makes the document its act, and the holder's eighteen-digit resident identity number.
