Municipal registries and the national register
Salvadoran births are registered by the municipal civil registry where the birth occurred, and certifications are issued from that municipality's books, with the Registro Nacional de las Personas Naturales maintaining the national identity register alongside. A certification is a transcription of the book entry, so its layout follows the municipality and the period rather than a single national template.
Older certifications are narrative: a paragraph of continuous text reciting who appeared, when, and what was declared, rather than a form with labelled fields. They are translated as the narrative they are, without being rearranged into a table that the original does not contain.
Marginal notes and what they record
Later events — a recognition of paternity, a correction, a marriage — are added as marginal annotations to the original entry, and a certification issued today carries the annotations that existed when it was printed. An older certificate in a drawer will not have them. If anything has changed since yours was issued, order a fresh one before translating.

What USCIS requires
The requirement is the same for every country: under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate.
