These are US bank statements
Puerto Rico is a US jurisdiction. Accounts held there are at US banks or US-regulated institutions, insured by the FDIC, denominated in US dollars, and identified by a routing number and an account number exactly as on the mainland.
There is no foreign currency, no IBAN, no apostille and no consular legalisation. An office treating a Puerto Rican statement as a foreign document has misunderstood where Puerto Rico is, and we would rather say so than charge for treating it as one.
When a translation is genuinely needed
Puerto Rico operates in Spanish and English, and some institutions issue statements, letters and certifications in Spanish.

A cooperativa statement is a regulated institution's statement. We name the institution and its supervisor rather than leaving a mainland reader to discount it.
Two surnames, and the identifiers
Puerto Ricans carry paternal then maternal surnames in the Spanish pattern, and mainland systems index the last word — the mother's name. We keep both and make the order explicit.
