Look at the record, not the wrapper
Many Peruvian birth certificates are reissued through RENIEC with a digital cover: a header with a QR code and a verification footer with a serial number and issue date. That cover can wrap very different underlying records, from a modern RENIEC form to a decades-old handwritten municipal entry.
The most common older format is a narrative partida from a municipal civil registry, recording the registered person, both parents, and the declarant. Modern RENIEC forms are structured, with fingerprints and a CUI number.
The second page is often part of it
Reissued certificates frequently include a second page certifying the copy as faithful to the registry record, and sometimes an apostille. Those pages are translated too: they are part of what you are submitting.
What USCIS requires
The requirement is the same for every country: under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate.
