Two formats, decades apart

A Mexican acta de nacimiento is issued by the Registro Civil of the state where the birth was registered, and what you hold depends on when you got it. Current certificates are a standardized national printout carrying a verification code, issued online or at a registry office and valid across the country. Older ones are entries transcribed from the registry book, frequently typed or handwritten, with the layout varying by state and decade.

Both are genuine. A recent printout of an old registration is the same record, reissued — which is worth knowing, because if your copy is faded or damaged, ordering a fresh one is usually easier than arguing about legibility later.

The CURP, and the two surnames

Current actas carry the CURP, an eighteen-character personal code built from the name, date of birth, sex and state of birth. It is reproduced exactly, character for character, and never reformatted.

The acta also records both parents with their apellido paterno and apellido materno, and the registered person carries two surnames of their own. Registration details — the oficialía, the book, the page and the entry number — are part of the record and are translated with it.

What USCIS requires

The requirement is the same for every country: under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note on your birth certificate is part of the document, so every one is translated.
