NADRA holds the name
In Pakistan the operative record is the NADRA database and the CNIC issued from it, not a civil registry birth entry. A change of name is effected by getting the NADRA record modified, and the reissued CNIC is the evidence.
Birth registration with the union council exists and produces a NADRA-issued birth certificate, but for an adult the CNIC is what offices read.
Affidavit, newspaper, then the record
The supporting route is a sworn affidavit before an oath commissioner and publication in a newspaper, submitted to NADRA with the application. The newspaper cutting is kept as part of the file.
So a Pakistani name-change packet is an affidavit, a cutting, and a before-and-after pair of CNICs rather than a single certificate. We translate each and identify what it is.
The CNIC number survives
The thirteen-digit CNIC number encodes the locality of registration, a family serial and a check digit including a gender digit, and it does not change with a name. An old CNIC and a new one sharing the number is the cleanest proof in the packet.
The B-Form or child registration certificate carries the same family number for children and is useful supporting evidence.
English, Urdu and the name order
Pakistani official documents are largely bilingual, with names printed in both Urdu and Latin script, and the two renderings frequently differ — Muhammad, Mohammad, Mohammed, Md. A difference in Latin spelling is usually not a change of name.
