The disability certificate, and the provincial councils
Pakistan issues a disability certificate through provincial councils for the rehabilitation of disabled persons, on the basis of a medical board assessment at a designated hospital.
Because the councils are provincial, the format, the assessment criteria and the issuing body differ between Punjab, Sindh, Khyber Pakhtunkhwa and Balochistan. We name the province and the council.
The percentage, and the employment quota
The certificate states a percentage of disability and a category, and the principal entitlement is placement against the statutory employment quota in public and larger private employment.
So the percentage is an eligibility position under Pakistani law rather than a clinical measure, and we reproduce it with the scale it belongs to.
The board, not a physician
Assessment is by a medical board at a designated hospital rather than by a treating physician, and the certificate names the board members and their designations.
A private physician's report is not a disability certificate in Pakistan however detailed, and we translate each document as what it says it is.

NADRA issues a special CNIC to holders of a disability certificate, which carries the entitlement markers, and the thirteen-digit number does not change.
