The identity card carries the patronymic, the passport does not
A Greek ταυτότητα — identity card — records the father's and mother's names, and it is a travel document within the EU. A Greek passport does not carry the patronymic.
So the two Greek documents for one person show different name data, and the identity card is the one that evidences parentage. A US office matching them needs to be told which is which.
Transliteration, and fifty years of family spelling
Greek passports follow a specified transliteration scheme that frequently differs from what a Greek-American family has used since arriving — Ιωάννης as Ioannis where the family has always written John or Yanni.
A great many apparent name changes in Greek-American files are exactly this and nothing more. The passport may carry an alternative spelling as a remark where the holder applied for one, and we translate that entry.
Married women keep their surname
Since the 1983 family law reform Greek women retain their birth surname as a matter of law, so a Greek passport shows it for life.
A married surname may be used socially and may appear on some documents as an addition. We reproduce both where the document shows both and say which is registered.
The AFM, the AMKA and the municipal register
The passport shows neither the AFM (tax number) nor the AMKA (social security number), and Greek civil status lives in a municipal register — the δημοτολόγιο and the ληξιαρχείο.
