The lixiarchiki praxi, and the municipal register
A Greek birth is recorded by the lixiarcheio, the registry office of the municipality, which issues the ληξιαρχική πράξη γέννησης — the birth registration act. Greece also keeps a separate municipal roll (dimotologio) recording family membership, and a certificate from it is a different document that offices sometimes ask for instead.

Transliteration, and gendered surnames
Greek transliterates several ways and the official ELOT system does not always match what is on a passport or what a family has used abroad for generations — Ioannis, Yannis and John can all attach to one person across documents. We follow the passport and note genuine differences rather than standardising.
Greek surnames are also gendered, so a daughter's form differs from her father's — Papadopoulos and Papadopoulou are the same family name. That reads as a mismatch on a US form and is not one, so the translation makes the structure explicit.
What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.
