The Impfpass is a yellow booklet and it is the record
Germany issues an Impfausweis or Impfpass — the yellow international vaccination booklet — and every dose is entered by the administering doctor with the date, the vaccine trade name, the batch number and the practice stamp.
It is the immunisation record and it is what a US civil surgeon needs for an I-693. It is a small booklet that people leave out of a scan, and the stamps are frequently handwritten over the printed entries.
Trade names, not generic names
German entries use the vaccine's trade name — Infanrix hexa, Priorix, Boostrix, Twinrix — rather than a generic description, and a US reader who does not know the product cannot tell which antigens a dose covered.
We give the trade name as written and the antigens it contains, because a hexavalent dose recorded under one product name is six entries on a US form.
Arztbrief, Befund and Entlassungsbrief
German clinical documents come in defined types: an Arztbrief or Entlassungsbrief is a letter from one physician to another summarising a treatment episode, a Befund is a findings report from an examination or a laboratory, and an Attest is a certificate for an employer or an authority.
They carry different weight and a US reader given three documents needs to know which is which. We name the type rather than translating all of them as "medical report".
ICD-10-GM, and abbreviations
German diagnoses are coded in ICD-10-GM, the German modification, with suffixes marking certainty — G for confirmed, V for suspected, A for excluded, Z for a residual state. A code without its suffix loses whether the diagnosis was actually made.
German clinical writing is also heavily abbreviated and Latinate. We expand the abbreviations rather than reproducing a page of initials. See our German page.
