The Cartilla Nacional de Salud
Mexico issues a Cartilla Nacional de Salud — a national health card issued by age group — which records vaccinations with dates and lot numbers alongside checkups and screening.
For childhood immunisations it is the record, and it is a small folded document that applicants routinely leave out. A US civil surgeon completing an I-693 needs the vaccination pages, and we translate every entry including the handwritten ones.
The expediente clínico belongs to the institution
The expediente clínico is regulated by a national standard and is held by the institution — IMSS, ISSSTE, the Secretaría de Salud or a private hospital — and the patient receives a resumen clínico, a discharge summary or a certificate.
Which institution holds it matters because IMSS, ISSSTE and state health services keep separate systems and a person's history may be split across them. We name the institution and the clinic number.
The physician's cédula profesional
Mexican medical documents carry the physician's cédula profesional — the federal professional licence number — and for specialists a second cédula for the specialty. That number is how the physician is verified.
We reproduce both rather than describing the document as signed by a doctor, because a US office assessing a foreign medical certificate checks the licence.
Vaccine names, and BCG
Mexican schedules use combinations and names that do not map directly onto the US list, and BCG is given at birth, which leaves a scar a US examiner may ask about and which is not a US vaccine at all.
