The Caderneta de Vacinação
Brazil issues a Caderneta de Vacinação — for children the Caderneta da Criança, which also records growth and development — and every dose is entered with the date, the lot and the vaccinating unit's stamp.
It is the immunisation record for an I-693, and Brazilian schedules include vaccines the US does not use routinely, notably BCG at birth and yellow fever. We reproduce every entry including the stamps.
The prontuário and the SUS card
The clinical record (prontuário) is held by the institution, and the patient carries a Cartão Nacional de Saúde with a CNS number linking them across the public system.
A patient receives a relatório médico, a discharge summary or an exam report rather than the prontuário itself. We name each document type and the issuing unit.
CRM, and how a Brazilian doctor is identified
Every Brazilian physician signs with their CRM — the regional medical council registration — written with the state abbreviation, as in CRM-SP 123456. The state matters: the registration is per state.
We reproduce the CRM with its state, because a US office verifying a Brazilian physician needs both to search the council register.
Exam reports, and the laudo
Brazilian imaging and laboratory reports are issued as a laudo — a formal signed findings report — separate from the images or raw results, and the laudo is the document with evidential weight.
