The laudo, and the CID code
The Brazilian document is usually a laudo médico stating the condition with its CID-10 code — the Brazilian rendering of the ICD — and the applicable legal basis. The code is the operative content and is reproduced exactly rather than translated into a description.
Brazil has no single national disability card of the Argentine kind, though it introduced a Cartão de Identificação da Pessoa com Deficiência and municipalities issue their own cards for transport and parking. We name the issuing body.
The Lei Brasileira de Inclusão and biopsychosocial assessment
Lei 13.146 of 2015, the Lei Brasileira de Inclusão, defines disability in biopsychosocial terms — the interaction of an impairment with barriers — rather than purely medically, and mandates assessment by a multiprofessional and interdisciplinary team.
A Brazilian assessment document may therefore contain functional and social findings a US reader does not expect in a medical certificate. Those are the statutory criteria, not padding, and we translate them.
BPC, and what the certificate is for
The BPC/LOAS is a means-tested continuous benefit for people with disabilities and older people, assessed by INSS through a medical and a social evaluation. The INSS assessment documents are a different set from a treating physician's laudo.

Isenções, and the vehicle exemption
Brazilian disability documentation frequently exists because of tax exemptions — IPI and ICMS relief on vehicle purchase, IPVA exemption, income tax exemption on certain pensions for specified conditions — each with its own required wording.
