The immunisation module, and the paper card
Georgian childhood immunisation is recorded in the national electronic immunisation module and on a paper card at the clinic, and a certificate can be issued from the electronic system.
Georgia gives BCG at birth, which is not a US vaccine. The electronic certificate is the cleanest document for an I-693 and many families do not know it can be issued.
The eleven-digit personal number
Georgian medical documents carry the eleven-digit personal number, which does not change with a name and is the reliable identifier across a Georgian file.

Mkhedruli, and no capital letters
Georgian is written in Mkhedruli, an alphabet with no upper and lower case at all, so a name that looks uncapitalised in the original is not informal and we do not invent capitalisation.
Romanisation is not standardised in practice, so one patient name appears several ways across documents. We reproduce the Georgian and note the renderings.
Soviet records, and the regions
Records from before 1991 were made under Soviet administration in Russian as well as Georgian, and a modern Georgian document may restate them.
Documents relating to Abkhazia and South Ossetia require the issuing authority to be named exactly as printed, without commentary. See our Georgian page.
