Dutch statements label each line bij (credit) or af (debit) — two-letter words easy to skim past — rather than using signs.

The tenaamstelling, and who the account belongs to
Dutch banks record a tenaamstelling — the name in which the account is held — and Dutch payment infrastructure checks it against the IBAN on every transfer, which makes a Dutch statement unusually reliable evidence of ownership.
The name may include a tussenvoegsel — van, de, van der — which is part of the surname and which the Netherlands does not use for alphabetisation. We reproduce the arrangement the bank uses.
IBAN only, and no branches
The Netherlands uses IBAN exclusively for domestic transfers and Dutch banks no longer operate a branch network in the sense a US reader assumes, so there is no branch number and no branch address on a statement.
An office asking for a branch address will not get one. We say so rather than leaving a field looking unanswered.
The decimal comma, and the bank declaration
Dutch numbers write a comma as the decimal separator and a point for thousands: € 1.234,56. Carrying the punctuation across is an error of a factor of a thousand.
