Croaticum, and the citizenship requirement
Croatian proficiency is certified principally through Croaticum, the Croatian language centre at the University of Zagreb, and other university centres, at CEFR levels.
Croatian naturalisation requires knowledge of Croatian, and a certificate is the usual evidence, so the document is frequently a legal precondition rather than an academic credential. We name the institution and the level.
The purist vocabulary
Croatia pursued a deliberate purist policy after 1991, coining Croatian replacements for internationalisms — zrakoplov, tipkovnica, redarstvenik — which are examined and expected.
So Croatian certification tests a vocabulary that diverges from Serbian and Bosnian in official and technical registers even where speech is mutually intelligible. That is what the certificate attests.

The three are mutually intelligible in speech, and a certificate in one is in practice evidence about the others, though institutions treat them as separate languages.
