The 2022 passport that several countries refused
Vietnam issued a redesigned navy-blue passport in 2022 that omitted the place of birth, and Germany, the Czech Republic and Spain suspended recognition of it because they could not distinguish holders with identical names and dates of birth.
Vietnam responded by adding the place of birth as an annotation in the remarks page rather than reprinting the books. So a Vietnamese passport may carry the critical field on a different page from where every other country prints it, and a reader checking only the biodata page will report it missing.
Nguyễn, and why the surname identifies nobody
Roughly two in five Vietnamese share the surname Nguyễn, and names run family name, middle name, given name — so Nguyễn Văn An is addressed as An and the first word is the surname.
A US system reading the last word as the family name inverts it, and a system matching on surname alone matches half the country. The full name with diacritics is what identifies the holder.
Diacritics, and what stripping them does
Vietnamese marks tone and vowel quality with diacritics and the machine-readable zone strips them, so Hà, Hạ and Há collapse into HA. Those are different names, not spellings of one.

The CCCD behind it
The CCCD — the twelve-digit citizen identification number — is printed on current passports and does not change; it replaced the nine-digit CMND, so a person may hold documents under both.
