The certificate, and the household book that is going away
A Vietnamese birth certificate (giấy khai sinh) is issued by the people's committee of the commune or ward where the birth was registered. Alongside it, families long relied on the household registration book (sổ hộ khẩu) to prove relationships — a paper book listing everyone in a household.
Vietnam has been retiring the paper household book in favour of an electronic population database, so applicants increasingly hold a confirmation of residence information rather than the book itself. Both are translated; tell us which you have, because they look nothing alike.
Diacritics are part of the name
Vietnamese marks tone and vowel quality with diacritics, and Nguyễn, Nguyên and Nguyện are different words. Stripping them — which foreign systems do constantly — produces a different name, and a file where the same person appears with and without them is a file with a discrepancy in it.

What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.
