The internal passport became a card in 2016
Ukraine replaced the Soviet-style internal passport booklet with an ID card in 2016, so a Ukrainian may hold a booklet, a card, or both. The internal document and the foreign travel passport are separate documents with separate numbers.
The booklet carries residence registration stamps and marriage entries; the card does not print residence, which is held in the state register and evidenced separately. We name which document we are translating.
The patronymic is in one and not the other
Ukrainian names have three parts — given name, по батькові (patronymic from the father's given name), and surname. The internal documents carry the patronymic; the foreign travel passport generally does not.
So the two Ukrainian documents for one person show different numbers of name elements. That is the form, not an inconsistency, and a US office matching them needs to be told.
Transliteration changed in 2010
Ukraine adopted an official Latin transliteration in 2010, replacing Russian-derived spellings — Сергій became Serhii where it had been Sergey.
A Ukrainian who left before 2010 and renewed afterwards has a genuinely different Latin spelling with no legal change behind it. That distinction is frequently what a packet turns on, and we state it.
Wartime documents, and consular issue
Since 2022 many Ukrainians hold passports issued at consulates abroad, documents reissued from the central register where local offices were inaccessible, and records restored after loss.
