The PESEL is printed on it
A Polish passport prints the PESEL — the eleven-digit national identification number encoding the date of birth and a sex digit — which does not change with a name and is the identifier every Polish institution uses.
That makes a Polish passport unusually easy to match against other Polish records, and we reproduce the number exactly including leading digits that spreadsheets routinely eat.
The dowód osobisty travels in the EU
The dowód osobisty — the Polish identity card — is a travel document within the EU and for a defined set of other countries, so a Polish client may present a card rather than a passport.
They are different documents with different validity and different data. We name which one you sent.
Surnames are gendered, and the passport shows it
Polish surnames in -ski, -cki and -dzki take feminine forms in -ska, -cka and -dzka, so a father's passport reads Kowalski and his daughter's Kowalska.
One family name. A US system treating them as different surnames creates a mismatch no document will explain, so we state the relationship in the translation.
Diacritics, and the machine-readable line
Polish uses ą, ć, ę, ł, ń, ó, ś, ź and ż, and the machine-readable zone strips them — Łukasz becomes LUKASZ, Wałęsa becomes WALESA.
