When the USPTO requires a translation
Under 37 CFR 1.52(d), a nonprovisional application filed in a language other than English requires an English translation of the application, a statement that the translation is accurate, and a processing fee. A later application claiming priority to a non-English provisional runs into the same requirement at that point.
Translations also come up for foreign priority documents, for non-English prior art cited in an information disclosure statement, and in litigation and opposition proceedings where what a foreign reference actually says is the entire dispute.
The claims are not prose
A patent specification can absorb a loose sentence. The claims cannot: they are the legal scope of the monopoly, and a word moved or a term rendered two ways changes what is protected. Claims are translated literally, with their structure and dependencies preserved, and every technical term is held identical between the claims, the specification and the abstract.
Figure labels, reference numerals and the sequence of drawings are translated in place so that the numbers in the text still point at the parts they name.

A standard translation is for business or personal use where certification is not needed, such as CVs, client intake forms or marketing material, and is billed at $0.10 per word.
