The Tribunal Electoral runs civil registration
Panama's Registro Civil is a directorate of the Tribunal Electoral, which also issues the cédula. As in Costa Rica and Nicaragua, the electoral authority is the correct issuer of a birth certificate and not a substitute for one.
A change of name is obtained judicially in the ordinary case, and a correction of an error may be made administratively by the Registro Civil. Which route produced your document is on its face, and we name it.
The cédula encodes the province
A Panamanian cédula is written as a province prefix, a tomo and a folio — 8-123-456, where 8 is Panama province. Prefixes such as PE, E and N mark people born abroad to Panamanian parents, foreigners and naturalised citizens.
So the number states where the person was registered and their status category on its face, and we reproduce the prefix rather than dropping it as punctuation.
Marginal notes on the birth entry
The change is recorded as a marginal note against the birth inscription, and a certificate issued afterwards carries it. Panama issues certificates electronically with a verification code, and offices increasingly check the document online.

Two surnames, and the Canal Zone
Panamanians carry paternal then maternal surnames, and we keep both and make the order explicit.
People born in the former Canal Zone before 1979 may hold US-issued records alongside Panamanian ones, with names recorded in the US pattern. A packet mixing the two is not inconsistent, and we name the issuing authority for each. See our Spanish page.
