The Ley de Paternidad Responsable
Costa Rica's Ley de Paternidad Responsable created an administrative procedure with no equivalent in the region: where a mother names a man as the father at registration, the Registro Civil notifies him, and if he does not object within a stated period he is registered as the father.
If he objects, a DNA test is arranged at state expense and the result determines the registration. So a Costa Rican child may acquire a registered father without the father ever signing anything, and that is the statute rather than an irregularity.
What the resulting document looks like
The outcome is an administrative registration rather than a court order or a notarial deed, and the birth certificate simply shows the father.

The civil registry sits inside the Tribunal Supremo de Elecciones, which also issues the cédula and operates the notification procedure.
