The registry and the identity number are the same system
Costa Rican civil records are kept by the Registro Civil, which sits under the national electoral authority, and the same institution issues the identity document. A Costa Rican's cédula number derives from the birth registration itself — the province, the book and the entry — so the identity number and the birth record are two views of one thing.
That makes the number worth reproducing with care: it is not an arbitrary identifier, and a transposed digit points at a different registry entry. It is carried across exactly as printed.
Certifications and annotations
What is issued is a certificación drawn from the registry, showing the registered person, the parents and the registration reference. Current ones are printed from the registry system and may carry a verification code; older ones are transcriptions from the books.
Marginal annotations recording later events are translated with the entry. Both surnames are carried across in the order the document gives them.
What USCIS requires
The requirement is the same for every country: under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate.
