The child has to consent
A Thai father acquires no legal rights over a child born outside marriage unless he legitimates the child, and registration of legitimation at the district office requires the consent of both the mother and the child.
The child consenting is unusual and it is the point: where the child is too young to consent, the registration route is closed and a court order is required instead. A US reader expecting a parental process finds the child holds a veto.
Three routes, three documents
Legitimation happens in three ways: the parents marrying, registration at the district office with both consents, or a court order. Each produces a different document.
We name which route the instrument records, because the consequences and the evidence differ. Registration produces a certificate from the amphoe.
The house registration, and the surname
The tabien baan, the house registration book, records the household and is updated after legitimation, and the child may take the father's surname.
So a Thai filiation packet is the legitimation document plus the house registration and the birth certificate, and applicants routinely send one of the three.

Thai documents are dated in the Buddhist Era, 543 years ahead of the Gregorian calendar, and we convert each date and show both.
