The municipal administration decides
A change of personal name in Serbia is granted by the municipal or city administration under the Family Law, available to anyone over fifteen with capacity, and refused in defined cases such as an outstanding criminal proceeding.
The decision is recorded in the matična knjiga rođenih, the register of births, and a new izvod issued afterwards shows the current name.
Two alphabets, one name
Serbian is written in both Cyrillic and Latin and both are official, so a person's name appears as Ђорђевић on the register and Đorđević on a passport. Those are the same name in the same language, not two names and not a change.
This is the single most common false alarm in a Serbian packet, and we state the equivalence explicitly rather than leaving a US reader to conclude the documents do not match.
JMBG, and what it encodes
The JMBG is the thirteen-digit unique citizen number encoding the date of birth, the region of registration and sex, and it does not change with a name. It is the strongest cross-check available.
People registered in the former Yugoslavia carry a JMBG issued under that system, and the regional digits may point to a region now in another country. That is where the person was registered, not a defect.
Izvod, and the international form
Serbia issues birth extracts on a domestic form and on the multilingual international form under the Vienna Convention, which carries standard field numbers and prints headings in several languages.
