A judicial route, and RENIEC records it
Peru requires a judicial authorisation for a change of name in the ordinary case, under the Civil Code, and the resulting resolución judicial is then recorded by RENIEC against the birth entry.
Simple corrections of a spelling error follow a separate administrative rectificación route, which is a different document with a different legal basis. A reader who conflates them misreads what was actually decided.
The anotación on the acta
RENIEC enters the change as an anotación against the acta de nacimiento, and a copy issued afterwards shows it. Peru operates several generations of acta format at once — the old narrative municipal books, the RENIEC form-style actas, and the current tabular printouts — and the annotation sits differently on each.
We reproduce the annotation where it appears on the document rather than relocating it into a standard position, because the reader is comparing our translation against the original page.
The DNI, and reissue
The DNI is reissued under the new name and the eight-digit number is retained, so the old and new DNIs share a number. That is the cleanest evidence in a Peruvian packet that the two names belong to one person.
The DNI also carries a verification digit, and we reproduce the number exactly as printed including it.
Two surnames, and the registry office
Peruvians carry paternal then maternal surnames, and a name change may alter one, both or neither. The translation states which element changed rather than presenting a new full name as if the whole thing were replaced.
