The Registro Nacional de las Personas
Honduras keeps its civil registry at the RNP, the Registro Nacional de las Personas, which also issues the identity card. A change of name is authorised judicially and recorded by the RNP against the birth inscription.
The RNP conducted a national re-enrolment in recent years and issued a new identity document with a new numbering, so a Honduran packet frequently contains identity documents under two different number formats for one person.
The marginal entry, and the reissued certificate
The change is recorded as a marginal entry on the certificación de nacimiento, and a copy issued afterwards carries it with the date and the judicial instrument.
Honduran certificates state the tomo, folio and acta together with the registry office, and we transcribe all of them rather than summarising.
Reposición, and what it is not
Honduran files sometimes contain a reposición — the reconstruction of a registry entry lost or destroyed — which reads to an unfamiliar eye like a record created after the fact. It is a lawful procedure with its own instrument, and it is not a name change.

Two surnames, and the RTN
Hondurans carry paternal then maternal surnames, and we keep both and make the order explicit rather than letting a US form index the mother's name as the family name.
