Separation first, and the routes out of court
Italy historically required a period of legal separation before divorce, and older files therefore hold a separation decree and a divorce decree, which are different documents making different claims. The required period was shortened in 2015.
Italy also created out-of-court routes: negoziazione assistita, agreed between the spouses through their lawyers, and an agreement concluded before the ufficiale dello stato civile at the comune. Either produces a document that is not a court judgment, and the translation names the instrument accurately.
The annotation, and full copies
However the divorce was obtained, it is annotated on the marriage record at the comune. A copia integrale of the marriage act shows the annotation; the summary extract does not. Offices frequently want the annotated copy alongside the divorce document.

What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.
