After July 2010, the court decision is the document
This is the thing worth knowing before you go looking for a certificate that does not exist. Where a Ukrainian marriage was dissolved by a court after 27 July 2010, the document that proves the divorce is the court decision that entered into legal force — DRATS does not issue a divorce certificate for it. Asking the registry for one will not produce anything.
Where the dissolution was registered administratively by DRATS instead — available by mutual consent without minor children — there is a divorce certificate (свідоцтво про розірвання шлюбу). Which of the two you have depends on how the marriage ended and when.
The date it entered into legal force
On a court decision the operative date is not the date it was handed down but the date it entered into legal force, and the two are different days. Ukrainian decisions carry a note or a separate certification confirming it, and that is frequently the page an office actually needs. Send every page.
Decisions and certificates for use abroad are apostilled by the Ministry of Justice; the apostille is translated with the document. Names carry patronymics and gendered surnames — see our Ukrainian birth certificate page.

Can I still get proof of my former surname?
An extract from the State Register of Civil Status Acts can confirm it. That is a different document from the marriage certificate, which is not reissued after a dissolution.
