More than one certificate, depending on who is asking
Colombia issues several distinct background certificates and processes ask for different combinations of them. The Policía Nacional certificate is the one usually meant by "police record" and is generated online with a verification code. Separately, the Procuraduría issues a certificate of disciplinary record and the Contraloría one of fiscal responsibility, and some procedures — particularly professional and public-sector ones — require those as well.

Generated online, and dated
These certificates are produced from the issuing body's system with a generation date and a verification code, and they are point-in-time documents: the date on the face is the date the search was run. Where a process requires a recent certificate, it is the generation date that counts.

What USCIS requires
The requirement is the same for every country: under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note on your police record is part of the document, so every one is translated.
