The Cancillería issues it, and the cédula is on it
Colombian passports are issued by the Ministerio de Relaciones Exteriores and its departmental offices, and through consulates. The biodata page carries the cédula de ciudadanía number, which is the identifier every other Colombian document uses.
For people born from the 2000s the cédula number is the NUIP assigned at birth registration, so the passport number links straight back to the civil registry entry. We reproduce it exactly.
Two surnames, and the passport fields
The passport prints both surnames, paternal then maternal, and a US system reading the last word indexes the mother's name. We keep both and make the order explicit.
Where a Colombian has used only the paternal surname in the United States for years, that is a usage difference and not a change of name — though a Colombian change of name is made by escritura pública and annotated on the registro civil, which is a real instrument we name when it appears.
The registro civil behind it
Colombian civil registrations are made by the Registraduría, by notaries and by consulates abroad, and all three are valid. Any change of name or filiation is a nota marginal on the registro civil de nacimiento.
Short extracts sometimes omit the marginal notes, so where the passport and an older document differ, ask for the full copy before ordering a translation of the wrong one.
Apostille, and documents for the United States
Colombia is a party to the Hague Apostille Convention and apostilles are issued electronically by the Cancillería with a verification code, so an apostille on a Colombian document is a digital record rather than a sticker.
