Since 2022 you can do it at the cartório
Brazil changed this substantially with Lei 14.382 of 2022: an adult may change their name once, directly at the cartório de registro civil, without a judge and without stating a reason. Before that it took a court order outside narrow exceptions.
So a Brazilian name-change document dated 2023 may be an extrajudicial act at a notary office, while one dated 2019 is a judicial sentença. Both are valid and they look nothing alike.
Averbação, and the new certidão
The change is recorded as an averbação against the original birth entry, and the certidão de nascimento reissued afterwards prints the averbações in a block, each with its date and the act that produced it.
Brazilian certidões issued since the 2010s use a national standard layout with a 32-digit matrícula encoding the cartório, the book, the page and the entry. We transcribe that number in full because it is how the record is verified.
CPF stays, and the rest does not
The CPF number survives a change of name — the number is permanent and only the registered name against it is updated — which makes it the most useful single identifier in a Brazilian packet.
The RG is issued by a state authority and reissued separately, the CNH by DETRAN, the passport by the Polícia Federal. Each carries its own number and its own issue date, and a packet routinely mixes documents from before and after the change.
The social name, and the 2018 provimento
Brazil has recognised the nome social in administrative use, and CNJ Provimento 73 of 2018 allowed transgender adults to alter name and registered sex directly at the cartório. The averbação in those cases is worded to that effect.
