Reconocimiento, and where it can be made
Venezuelan acknowledgment may be made in the birth registration itself, by a later appearance before the civil registry, by public instrument, or in a will, and the 2009 Ley Orgánica de Registro Civil governs the recording.
A later acknowledgment is entered as a nota marginal on the acta de nacimiento, and a certified copy issued afterwards carries it. That annotated copy is what a foreign office should be sent.

Venezuelan child protection law asserts the child's right to know their filiation, and the protection councils and courts may act to establish it where a father does not acknowledge.
A judicial declaration of paternity is a different instrument from a voluntary acknowledgment, with different consequences if later contested. We name which the document is.
The surname, and the two surnames
Acknowledgment adds the paternal surname in first position, so a child registered with the mother's two surnames comes to carry the father's and the mother's.
A US form indexing the last word indexes the mother's name in both cases, which makes the change look smaller than it is. We keep both and make the order explicit.
