The administrative office decides
A change of personal name in Croatia is granted by decision of the county administrative office under the act on personal names, and recorded in the matica rođenih, the register of births.
A spouse may also take or revert to a surname on marriage or divorce by declaration, which is a different route with a different instrument. We name which was used.
Izvadak, and what it shows
The izvadak iz matice rođenih is the birth register extract, and Croatia now issues it electronically through the e-Građani system with a verification mechanism. The extract shows the current name and the register notes.
Where a document is an electronic issue we describe it as such and transcribe the verification reference, because an office checking the document online needs it.
OIB, and the diacritics
The OIB is the eleven-digit personal identification number used for individuals and companies alike, and it does not change with a name. It is the cleanest identifier in a Croatian packet.
Croatian uses č, ć, đ, š and ž, and stripping them changes the name — Ćosić becomes Cosic. We reproduce the Croatian and say that the stripped form is the same name.
Yugoslav-era records
Births registered before 1991 were recorded under Yugoslav administration, and a Croatian register book may hold entries made by an authority that no longer exists. The reissued extract is a Croatian document restating that entry.
