Voluntary acknowledgment at the hospital
As on the mainland, an unmarried father may complete a voluntary acknowledgment of paternity at the hospital at birth or later at the Registro Demográfico, and the birth certificate then names him.
That form is a US instrument with US consequences, including a limited period to rescind it. We name it as what it is.
Two surnames, and what acknowledgment changes
Puerto Rico follows the Spanish naming pattern with paternal then maternal surnames, so acknowledgment changes the child's surname arrangement in a way a mainland record would not.
A mainland system indexing the last word indexes the mother's name. We keep both and make the order explicit.
The 2010 invalidation, and Spanish
Puerto Rico invalidated every birth certificate issued before 1 July 2010, so an older annotated certificate has no legal effect and needs replacing before anything else.
